Your donation to this fundraiser will support the creation of the Communities of Care program: a local project that will engage 50 trained volunteers to support 30 older adults through ongoing, repeated points of contact. Volunteers will assist with simple activities of daily living, provide social connection, and facilitate structured conversations about life goals, caregiving preferences, and end-of-life planning. 

The program advances four primary goals:

  1. Increase agency and independence for older adults living without adequate social support.

  2. Reduce loneliness and isolation through intentional communities of care that foster durable trust bonds and mutual aid.

  3. Increase volunteer confidence and caregiving capacity, equipping community members with the skills and support needed to provide nonmedical assistance.

  4. Achieve sustainability by embedding leadership, peer facilitation, and community ownership into the model.

The program will reduce isolation, strengthen informal caregiving networks, and increase preparedness for aging-related transitions. Expected outcomes include increased social support, documented care preferences, and sustained, trust-based communities of care that endure across participants’ lifespans.
